Standard Choking Response: First-Aid Protocols Come First

When a choking emergency occurs in a school, the immediate response follows established Basic Life Support (BLS) protocols. For a child over one year of age or an adult, this means a combination of back blows and abdominal thrusts, commonly known as the Heimlich maneuver. These techniques are the first line of response and should always be attempted immediately when a person shows signs of complete airway obstruction.

Staff training in BLS choking response is the foundation of any school choking preparedness plan. What LifeVac Is and How It Works

In March 2026, the U.S. Food and Drug Administration granted LifeVac De Novo classification as a Class II medical device, establishing a formal regulatory category for suction anti-choking devices used as a second-line treatment. This made LifeVac the first and only device of its kind authorized for marketing and distribution in the United States. The FDA defines this device type as one that uses suction to help resolve choking in victims experiencing complete airway obstruction, and it is authorized for use after standard BLS choking protocols have been attempted without success.

LifeVac works through a one-way valve system that creates suction to pull an obstruction out of the airway rather than forcing it further down. The device is non-powered, non-invasive, and designed for single use. It is authorized for use on adults and children at least one year of age and may be administered by a layperson who is at least 18 years old. LifeVac does not replace calling 911, which should happen immediately when any choking emergency is recognized.

The LifeVac device itself does not expire. The manufacturer recommends replacing the masks every two to three years.

Why Schools Are a Specific Use Case

The school cafeteria is the highest-risk location in the building for choking. Students eat quickly, talk while eating, move between tables, and are frequently distracted. These behavioral factors are specifically identified by the American Academy of Pediatrics as conditions that elevate choking risk. Lunch periods in many schools involve large numbers of students eating simultaneously with limited adult supervision per student.

Beyond the cafeteria, school nurses encounter choking-adjacent emergencies, athletes occasionally choke on food or drink in and around gymnasiums, and students with certain swallowing difficulties face elevated risk throughout the school day.   • Main or principal's office: A unit at the administrative center of the building serves as both an accessible backup and a training device for administrative staff who may not be near the cafeteria during an emergency.

Schools with multiple buildings, very large campuses, or specialized programs should consider additional units beyond the base four. A wall-mounted case in high-traffic hallways or common areas extends coverage for buildings where these four locations do not reach all occupied spaces.

The Legislative Landscape

Texas passed HB 549, known as the Westyn Bryan Mandrell Act, in June 2025. Signed by Governor Greg Abbott, the law requires every public school campus in the state to have non-invasive airway clearance devices available in the locations where students eat, effective with the 2025-2026 school year. The law was named for a young child whose death from choking at school motivated the legislation.

Texas is the first state to mandate airway clearance devices in schools, but the movement toward treating these devices as standard school safety equipment, comparable to AEDs, is building nationally. Administrators in states without current mandates who add LifeVac to their emergency preparedness programs are ahead of a regulatory curve that is likely to continue moving in this direction.
